چکیده

QUESTION A few years ago I suffered from pulmonary emboli. My physician recommended I use dalteparin during this pregnancy although, during my previous pregnancy, I had received subcutaneous heparin injections three times daily. Is dalteparin the same as heparin? ANSWER : Based on the best available evidence from mostly small prospective case series, retrospective reports, and placental perfusion studies, low-molecular-weight heparins (LMWHs), such as dalteparin, are a safe and convenient alternative to heparin during pregnancy for both mothers and fetuses.

Fetal and neonatal effects of anticoagulants used in pregnancy: a review.

There are several relative (promising regarding a reduction in placenta-mediated complications such as preeclampsia) and absolute (e.g. a recurrent or recent thromboembolic event, mechanical heart valves) reasons for use of anticoagulant drugs during pregnancy. Warfarin readily crosses the placenta because of its low molecular weight, and is associated with a distinctive embryopathy known as fe...

Population pharmacokinetics of enoxaparin during the antenatal period.

BACKGROUND The optimal dosing strategy of low-molecular-weight heparins for the treatment of antenatal venous thromboembolism is not known. The physiological changes associated with pregnancy alter the pharmacokinetic profile of low-molecular-weight heparins, which has led to controversy and subsequent variation in practice, when pregnant women with venous thromboembolism are treated with low-m...

Oligosaccharide mapping of low molecular weight heparins: structure and activity differences.

Low molecular weight heparins from a variety of commercial sources were examined. These had been prepared by several methods including peroxidative cleavage, nitrous acid cleavage, chemical beta-elimination, enzymatic beta-elimination, and chromatographic fractionation. The molecular weight and polydispersity of these low molecular weight heparins showed greater differences than were observed f...

Incidence of hypersensitivity skin reactions in patients on full-dose low-molecular-weight heparins during pregnancy.

BACKGROUND Low-molecular-weight heparins (LMWH) are the most commonly used anticoagulants for the treatment and prophylaxis of venous thromboembolism in pregnancy. Hypersensitivity skin reactions associated with the use of LMWH are frequently seen, but are probably underreported. OBJECTIVE To evaluate the incidence of hypersensitivity skin reactions due to the use of LMWH in pregnancy, and th...

Molecular weight of low molecular weight heparins by 13C nuclear magnetic resonance spectroscopy.

Heparin and low molecular weight heparins are polydisperse polysaccharides with a degree of polymerization ranging from 4 to approximately 40. The determination of their average molecular weights has traditionally relied on size exclusion chromatography involving the use of oligosaccharides of known size and molecular weight as standards. 13C NMR spectroscopy is applied for the first time to ob...
